GLIN’s Physician Advisors prepare physicians for a future of value-based care, and directs physicians and staff to available resources that will assist in practice transformation such as, population health management, Patient Centered Medical Home (PCMH) and Patient Centered Specialty Practice (PCSP) recognition, self-reporting, incentive opportunities, and customized training for administrative staff.
GLIN’s behavioral health program, Integrated Care for Kids (InC/K), aims to increase access to quality mental health care through integration with primary care. Mental health concerns are an underappreciated and undertreated reality in healthcare. Real problems cannot be addressed because of limitations in resources. GLIN has taken the lead in addressing these concerns by developing a unique program where GLIN primary care physicians can easily collaborate with mental health providers to help meet patient needs.
