Great Lakes Integrated Network (GLIN) is going beyond basic care to create and support health programs integrated across the entire continuum of care and is recruiting for a Registered Dietitian/Certified Diabetes Care and Education Specialist to work at several physician offices in our Northtowns & Southtowns locations.
The Registered Dietitian Nutritionist/CDCES, in collaboration with the multidisciplinary team and in accordance with approved protocols, provides Diabetes Self-Management Education and training for patients with diabetes. The RD/CDCES will also provide outpatient nutrition counseling and education to clients/families with a variety of needs and diseases. This would include but is not limited to: diabetes, heart disease, autoimmune diseases, obesity, food allergies, gastrointestinal disorders, dysphagia, underweight and metabolic disorders for our pediatric, adult, and geriatric populations. This includes coordinating care, assessing and identifying risk factors, negotiating care plans with the patient, education (group and/or individual), monitoring and communicating outcomes with other providers and health care team. Candidate must also have experience with enteral and parenteral nutrition and providing recommendations as needed.
