Great Lakes Integrated Network (GLIN), an affiliate of Kaleida Health and Erie County Medical Center Corporation, is a Buffalo-based, physician-led independent physicians association (IPA) consisting of approximately 600 physicians in the areas of primary and specialty care. With a focus on the efficient delivery of Primary Care and other specialty care, GLIN builds collaboration, quality, efficiency, and value into a dynamic healthcare delivery system.  GLIN is seeking a Registered Dietitian Nutritionist/CDCES to join our team.

The Registered Dietitian Nutritionist/CDCES, in collaboration with the multidisciplinary team and in accordance with approved protocols, provides Diabetes Self-Management Education and training for patients with diabetes. The RD/CDCES will also provide outpatient nutrition counseling and education to clients/families with a variety of needs and diseases. This would include but is not limited to diabetes, heart disease, autoimmune diseases, obesity, food allergies, gastrointestinal disorders, dysphagia, underweight and metabolic disorders for our pediatric, adult, and geriatric populations. This includes coordinating care, assessing and identifying risk factors, negotiating care plans with the patient, education (group and/or individual), monitoring and communicating outcomes with other providers and health care teams. The candidate must also have experience with enteral and parenteral nutrition and provide recommendations as needed.

Duties and Responsibilities (including but not limited to)

  • In collaboration with the PCP and multidisciplinary team, identify and prioritize strategies to assist patients with Diabetes Self-Management education, and strategies.
  • Understanding of the pathophysiology and medication management of diabetes, dyslipidemia, hypertension, gastrointestinal issues, IBS/IBD, Celiac disease, food allergies, Obesity, oncology, metabolic disorders, and basic concepts of nutrition care coordination, self-management and motivational techniques.
  • Using self-management-training techniques, assess, plan, and implement patient and family education in an individual and/or group setting. Education includes but is not limited to self-blood glucose monitoring, medications, diet, exercise, emotional adjustment, and coping skills, and risk reduction.
  • Develop meal plans and educational materials.
  • Assist in teaching the ADCES accredited diabetes classes regularly. This may also include evening classes.
  • Work closely with the lead dietitian and other dietitians on staff to help ensure that department goals are being met.
  • Attend health fairs and engage in other community events to promote nutritional services.
  • Participate in monthly nutrition services staff meetings to ensure congruity in care and consistency within the department as well as stay abreast of changes within the company.
  • Other projects and duties as assigned.

 Qualifications:

  • Bachelor of Science in Nutrition Required, Master of Science is preferred.
  • 2-3 years of experience in outpatient nutrition counseling.
  • 2-3 years of inpatient clinical experience preferred.
  • Registered Dietitian with current licensure and registration in NYS (CDN).
  • Advanced training in diabetes preferred (certification by the National Certification Board for Diabetes)
  • Educators (CDCES) or a minimum of comprehensive diabetes training or equivalent with evidence of a minimal 15 CUE’s annually required.
  • Required to take CDCES exam within 6 months of the date of hire if does not currently hold that credential.
  • Advanced certificates of training or certifications preferred ( examples: FODMAP, Plant-Based, Oncology, Renal)

Knowledge and Skills:

  • Pathophysiology and management of diabetes, lipids, hypertension, gastrointestinal issues, IBS/IBD, Celiac disease, Chron’s, metabolic disorders, and basic concepts of nutrition care coordination, self-management and motivational techniques.
  • Knowledge of the Nutrition Care Process & Malnutrition Diagnosing Criteria.
  • Experience with writing nutrition care plans for patients on enteral and parenteral feeds.
  • Proficient in discussing lab results with patients.
  • Familiarity with vitamins, minerals, and supplements.
  • Strong leadership skills, ability to work independently.
