Great Lakes Integrated Network (GLIN), an affiliate of Kaleida Health and Erie County Medical Center Corporation, is a Buffalo-based, physician-led independent physicians association (IPA) consisting of approximately 600 physicians in the areas of primary and specialty care. The mission of GLIN is to transform the healthcare experience in Western New York, promoting high-quality, equitable, and efficient healthcare through innovation and collaboration. With a focus on the efficient delivery of Primary Care and other specialty care, GLIN builds collaboration, quality, efficiency, and value into a dynamic healthcare delivery system. GLIN is seeking a Quality Outreach Coordinator to join our team.

The primary focus of the Quality Outreach Coordinator is to provide preventative health service education and outreach to patients to facilitate the completion of screening and disease management testing.  This will ensure quality care to patients and support the success of the GLIN Quality programs.

 

Duties and Responsibilities (including but not limited to)

  • Provide patient education by phone, patient portal and by mailing educational materials to promote optimal health and compliance with Quality health metrics
  • Research patient charts for preventative health services test results and correct document codes as needed
  • Create quality triages/communication and document preventative health quality metrics that are needed or need to be completed by patients
  • Ensure patients with disabilities or those without health insurance have an awareness of how to complete their preventative health screening tests
  • Upload patient test results and exclusion criteria onto insurance portals to close insurance company gaps in care
  • Retrieve medical information from HEALTHeLINK, Infoclique, and other sources for upload into the patients’ charts
  • Complete Insurance Appointment Agenda forms and attach all appropriate medical documentation
  • Contact patients to help close gaps in care by offering to schedule patients on the Mammogram bus
  • Encourage patients to complete Colorectal Cancer Screening and facilitate scheduling as necessary
  • Document all patient interaction in appropriate Electronic Medical Record (EMR) in an accurate and timely manner
  • Deliver exemplary customer service in order to provide a positive patient experience across the organization
  • Perform other duties assigned by management

Education

  • Medical Assistant or LPN schooling or certificate required

Experience

  • Minimum 2 to 4 years in healthcare setting, primary care experience preferred
  • EMR experience required
  • Excellent communication skills both verbal and written
  • Ability to handle multiple tasks and priorities
  • Exceptional organizational skills
  • Outstanding customer service skills
