Clinical Pharmacist

The Great Lakes Integrated Network's pharmacy team is seeking a Clinical Pharmacist to work in a multidisciplinary environment, collaborating with our network providers, health system and contract payers to assist in building a superior care pathway to improve the overall health of patients.The position will be involved in the development of disease management programs and will implement comprehensive medication management services across Great Lakes Integrated Network/Optimum Physician Alliance's practices.

The Clinical Pharmacist will be responsible for working with physicians, pharmacists, nurse practitioner, physician assistants and other healthcare professional as part of a multidisciplinary care team in an effort to improve patient care, lower healthcare costs and assist with the implementation of payer based quality improvement initiatives. They will be expected to provide sound pharmacotherapeutic recommendations to providers based on current practice guidelines and participate in the development of pharmacy based initiatives and expansion opportunities. 

Job Responsibilities:

  • Working with the Director of Pharmacy Services to evaluate for opportunities to integrate clinical pharmacy support within OPA/GLIN’s network practices
  •  Supporting OPA/GLIN’s goal of engaging network providers and embedding pharmacy services into new practice environments
  • Providing comprehensive medication management services to patients via in person consultation or via telehealth
  • Ensuring appropriate and cost effective drug therapy use, areas of focus may include but not limited to evaluating and addressing drug-drug interactions, duplicate therapy, low and/or high doses of medications, opportunities to reduce medication cost
  • Serving as a drug information and medication safety resource expert for the network and clinical team
  • Assisting with the provision of Medication Therapy Management (MTM) services to all eligible patients attributed to the Great Lakes Integrated Network (GLIN)
  • Participating in the education of staff, students, interns, residents and other healthcare providers; acts as a preceptor and mentor where applicable

Qualifications:

Education:

  • Must possess a Doctor of Pharmacy (Pharm.D.) from an ACPE accredited School of Pharmacy
  • Must have completed a PGY-1 residency in an applicable area of practice, ambulatory care experience preferred
  • Current, active license to practice pharmacy in New York State, required

Experience:

  • Minimum 1 year professional experience in a clinical setting with at least 50% of time spent involved in direct patient care
  • Diabetic Educator certification (preferred)
  • Board Certification (BPS) certification (preferred)

Specialized Knowledge & Skills Needed:

  • Ability to analyze/interpret and utilize results of various laboratory and microbiology tests and apply them to patient care in a clinical practice setting
  • Ability to effectively utilize clinical practice guidelines and evidence based medicine in everyday practice
  • Excellent interpersonal skills and the ability to convey educational material in an understandable manner to patients and their families
  • Ability to use hospital based information systems including Pharmacy, Laboratory, Patient Accounting and office systems
  • Ability to operate ancillary patient care information systems in conjunction with operation of patient testing equipment
  • Ability to communicate effectively both written and orally with Physicians, Physicians Assistants, Nurse Practitioners, Nurses and other allied health professionals
  • Must be able to work independently to meet job requirements
  • Must have the ability to travel within Erie County and the surrounding counties as required
