Care Coordinator- Care Management

Great Lakes Integrated Network (GLIN), formed as Optimum Physicians Alliance in 2012, is a Buffalo-based, physician-led independent physicians association (IPA) consisting of approximately 600 physicians in the areas of primary and specialty care. The mission of GLIN as a Clinically Integrated Network is to work as a community of providers to accelerate smart, patient-centered and value-based medicine, with the goal of building a healthier WNY community.  GLIN is seeking a Care Coordinator - Care Management to join our team.

The Care Coordinator (CC) is a team member who facilitates patient continuity of care and preventive care within the care management team. Under the clinical direction and oversight of the patient’s Primary Care Physician and Registered Nurse, the Care Coordinator navigates patients at risk by collaborating with the patient, family, physician, nurses, and other members of the healthcare team to serve needs and expedite appropriate, cost effective care.  They will engage patients, support the care team and take an active role in improving the health of patient population served by GLIN’s network providers.  They will collaborate with the care team in providing the patient with the tools necessary to make healthy lifestyle choices, adopt life-long healthy behaviors, and close gaps in care. 

Duties and Responsibilities (including but not limited to)

  • Address patient risk factors and/or obstacles to care
  • Facilitate engagement between PCP and patient
  • Demonstrated ability to influence patients while motivating them to change
  • Participation in required care coordination conferences
  • Maintain predetermined patient caseload
  • Take initiative in finding solutions to difficult and/or sensitive problems
  • Work as patient advocate
  • Performs additional duties as assigned

 Qualifications or Education, Training and Experience

  • High school or GED diploma
  • Customer service or related experience in a complex environment, healthcare customer service experience preferred
  • Two (2)+ years of work experience in one or more of the following settings: Acute Care, Ambulatory, Specialty, Physician Office, Care Management, Home Health Care or related
  • CAN/ LPN/MA preferred
  • Must have valid driver license and able to travel

Knowledge and Skills/Expected Competencies

  • Candidate must be able to follow workflows and work together with a multi-disciplinary care team within a variety of clinical and non-clinical settings
  • Multitasking and prioritization working in a high-volume environment with some supervision
  • Understanding of HIPAA and Confidentiality requirements
  • Excellent interpersonal and organizational skills
  • Compassionate and able to handle stress
  • Adaptable to new technologies and software
  • Proficiency in EMR system(s), Outlook and data entry experience preferred
